Default Placement Interview Checklist

I use this form when conducting the oral interview with prospective students. It helps me to identify their level and put them in the correct class.

it looks very thorough. there's one problem, at least on my computer, the column lines for 'Good' & 'Fair' are pushed way off the chart. is it messed up on your version or is it just my computer?

I'm not sure how you decide on the level of the students according to this checklist. Some of the grammar will not have been taught at all in lower levels of course. But I think some guidelines could be helpful.
