Warning form for students who are struggling in class

This form can be used to send home to the parents if a student is really struggling in class. It's good if you have situations where students go from one level to the next regardless of their real level. Send a warning home to the parents letting them know their child is having a difficult time in class and if that continues they will not be able to continue on to the next level.

It's in Word Doc format so you can easily edit it to fit your needs.

Simple, but effective. You can always tailor the form.

Personally, I prefer providing a checklist of problems. The bureaucratic appearance adds to the letter's power. Just a thought.

Every teacher, as you note, should have this type of form on hand to distribut to parents and students.
