Default Student Evaluation Sheet

I created a multi-student evaluation sheet in Excel. This has the same format as the attendance sheet that I posted, so yes, I know they look almost the same. I created this as a monthly form I can use to keep track of the average progress for a class of nine 12-year-olds that I teach. I am more than open to any suggestions for how to improve this form.

Thank you for sharing this valuable student evaluation sheet. I especially appreciate the way that you distinquish between social language development and academic language development.

I would love to see it. When I tried to download, it came through as code and I don't know how to read it. Any suggestions or is there a simple way to convert to adobe or word?
thanks for creating something so useful

If you use MS Excel, you should have no problem opening it. I'm not familiar with the import options on MS Word or Adobe, so I wouldn't know which direction to point you in with that. Sorry. :/
