it sounds like you have added duplicate images to your worksheet's picture list.
to fix it just open the worksheet up by clicking edit next to the name here: http://www.eslhq.com/worksheets/saved_worksheets.php
then click the checkboxes of the duplicate images.
after you have checked all the duplicates, scroll to the bottom and click 'Delete Selected'.
