I created a multi-student evaluation sheet in Excel. This has the same format as the attendance sheet that I posted, so yes, I know they look almost the same. I created this as a monthly form I can use to keep track of the average progress for a class of nine 12-year-olds that I teach. I am more than open to any suggestions for how to improve this form.
