Question how to eveluate oral reading?

Im looking for a FORMULA to do the evaluation. I have this one to evaluate the speed:
#of words student has read/90x60=#of words a student read per minute.
How can I evaluate the accuracy?
Perhaps I have this one:
#of words student has read-mistakes/#of words student has read x 60=average, BUT
the grade they get is too high, it's not fair for everybody, I mean some students really read very well, while other read like a student in 1st grade (I teach 5th and 6th) How can I evaluate them?
