Diagnostic is the adjective while diagnosis is the noun. So you would do a Diagnostic Test but (in ESL) could also talk about it as being a Language Diagnosis. I tend to use the two terms interchangeably.

Would a 'language diagnosis' deal with a medical, physical, or mental capacity problem? Same for 'diagnostic test'?

Both would seem so to me.
